I received one in March from a Courtyard in Jacksonville and one in February from the desk at a Marriott in Puerto Rico. Courtyard was on email.

I used to fill them out, but now do so on rare occasions.

I did submit the one to PR, as the service and the staff were exceptional. It is truly amazing when you run into two or three great staff members and the impression it gives of the entire hotel. The Puerto Rican Marriott has chosen their staff well, and then trained them well after that.
