Going back from Manhattan (eastbound), I'd assume it's a right on Van Dam after getting off the bridge and then I can get on the eastbound LIE?
At the end of Van Dam, you can make a sharp left (with the arrow) to the upperdeck or make a wider left to the lower deck. Just stay in the middle lane of Van Dam and you'll be fine.

Going back is a different story. It shouldn't be but it is. If I remember, you should see signs for 495. This you might seek somebody else's help. I'm always taking the LIRR out and driving back with my husband.

Do keep in mind that the road signs will not say 59th Street Bridge. They know the bridge as the Queensboro Bridge.
