I have had a similar problem with CO flights not crediting to WorldPerks. WP made the same demands for boarding passes, ticket numbers, receipts, notarized copies of birth certificates, DNA samples, etc...

I FedExed them a package containing only photocopies of my boarding cards and my E-ticket itinerary printouts (from continental.com). My miles were credited within the week.
