Visa's coverage is primary only if you
don't have collision and comprehensive insurance on your own car, or if you're renting outside the US.
Otherwise, if there's damage to the rental car, Visa will only pay up to the deductible on your collision/comp insurance (usually $500). If the claim is for a larger amount, your insurance company will pay the rest...hence "secondary" coverage from Visa.
