The 30 day window only applies to European flights. I remember last year I called CO to make reservations for flights from BOS to Bali which traveled along the following routing: BOS-EWR-HNL-GUM-DPS-GUM-HNL-IAH-BOS. During the reservation process, I told the agent that I was considering transferring Amex miles into my account to upgrade to BF on the flights. She booked me in BF all the way except for my DPS-GUM segment on the return. I then received a phone call a few days later from Onepass informing me that I had cleared the waitlist for First Class on that segment.

All of this transpired well in advance of the 30-day window, and before I had even ticketed the reservation! So, the long and the short of it is that the 30-day window only applies to Europe as far as I know.
