FA's do have aprons and kukui-nut leis. (Sometimes fresh flowers, though you can bet these aren't furnished by AA)
I must admit, you had me going there (it's far too early in the islands). One can always hope that AA would give some serious thought to their service, but I can't see any of this happening anytime soon. Further, AA would make several of these improvements on international 767 flights before any domestic flight, which includes Hawaii.
As far as the yield on the Hawaii routes, it varies during times of the year, though I know that the flights from LAX are profitable. The much longer ones from ORD-DFW I don't know about for sure, but every time I've taken them, it's been an full load.
