If I go through my list above, my one visit to the UA F lounge in ORD ticked most of those boxes. Can't remember about the champagne, but it was very nicely done inside (not like the RCC's) very quiet, nice view and a so-so array of food...
Perhaps its well received here on FT because it's consistent???

First and foremost, everytime I've used the KUL based MH F lounge, I've received nothing but exceptional service; everything from ordering and having a freshly brewed cuppacino personally delivered to my table right through to having a spacious and well stocked bar in the smoking room (nasty habit but it's nice to have a cold drink with my smoke

). It's the small and subtle things which I feel makes the difference between a good lounge and a mediocre lounge experience. I'm not talking about large discernible differences, just some small touches here and a smile there.
Of the 'western' airlines, I'd have to say that BA and QF lounge staff have been the most consistent in terms of friendliness and attentiveness. With that said, I've never received 'exceptional' service - just good service.
My past experiences with Lufthansa and AA were utterly abysmal. Not only did I feel like a third class citizen, which I'm sure some attendents felt I was, but was continually given the third degree, "Are you sure you can afford to be in here?"
Not wanting to initiate any heated debates, just merely stating my personal opinion.
