Porcão on Rua Vinicius de Moraes (corner Rua Barão da Torre), a few blocks from Everest Hotel. Also on the same street are Bar Vinicius de Moraes (restaurant and bossa nova place) and Bar Garota de Ipanema, supposedly the place where Vinicius de Moraes and Tom Jobim wrote the famous song, Girl from Ipanema.
If you want a quiet place where locals dine, go for Mistura Fina on Avenida Borges de Medeiros in Lagoa, a mere 10 reals ($4) cab ride from your hotel. They have samba/pagode/chorro (live) music on Sunday nights (open till midnight).
For a night of fun, go to Scenarium in Lapa but don't go before midnight since nothing happens before then. When clubbing, make sure you keep the piece of paper or card they hand you when you enter the club. All of your consumption is recorded on the paper (or card) and you settle your bill when you leave. You do not need to tip waiter/bartender since the tip will be included in the final bill.
Ipanema is mostly residential (read: few hotels); the vast majority of the hotels (read: restaurants, shops, ATMs, etc.) are located in Copacabana. If you're looking for an ATM, try Banco do Brasil machines marked "Saques" and look for Plus/Cirrus/Visa/Maestro sign. Not all banks, Banco Itau for example, accept foreign ATM cards.
I also recommend using cash instead of credit cards to avoid double swiping of cards (apologies for the insensitivity). Also, do what locals do before paying, check every item on your bill and if must use your credit card make sure the amount corresponds with the bill.
And a final thought: in Brazil, education is free and compulsary. Resist giving money to child beggars; it encourages them not to go to school.
