Correct and yes travel must commence from Egypt. OW airlines serving CAI are BA and IB. If Egypt is too much of an adventure for you, try Istanbul, where the prices for this type of ticket are also quite good.

I'm assuming you're asking about buying a DONEx (a business class ticket) and upgrading to First. As far as I know, you'll only be able to do this on any segments operated by AA offering a First cabin (D class in North America = First Class on 2-class flights).
