I had several rentals that were not being credited to my account last year as I had entered my EB# incorrectly in my Hertz profile. I found a link to the Hertz retroactive registration at the United website, and modified it for SK:

I filled in and submitted the form (you need the rental agreement # and the rental dates), and received an email response within one day. The points were successfully added to my EB account.
