The tracking form is, indeed, a written confirmation from the hotel that one's rate is a qualified. As for the card, the forms are set up to be machine-imprinted; however, my last stay the clerk wrote my info by hand on the form instead. One must be very firm - nay pushy - about the airline miles. Either employees are specifically trained to default to points, or few guests go that route. I said something to the effect of: "Could I have a tracking form for the airline miles please?" Sure enough - he scribbled in the point total instead.
