I really feel for people who are getting scrunched. Airlines or anything else really shouldn't subject people to such an experience.

I've had similar experiences but not on a plane.

Regarding Southwest's policy, the heavier person has to purchase 2 seats before flying. However, if the flight is not sold out, they'd get a refund afterwards.
