Don't ask about dimensions or alike. Usually the answers are rubbish.
Do not use that online form as they sometimes add it as additional sports equipment which will cause issues during check-in if you want to change it back to "sports equipment as part of the free baggage allowance".

The sports equipment should then appear in your booking when you scroll all the way down or in the app after you went into the booking details.
There should not be any red "X" after the flight in the sports equipment section. Or if there's one, then it should got away after a day. If not, you need to call again.

Once the check-in agent sees that you have a confirmed golf bag in your reservation, you'll most likely be fine (provided that no side of your bag is longer than 2m).

Notably, the size is not explicitly mentioned in this wording.

Here size is stated.
Obviously, a golf bag will never meet the normal 158cm (L+W+H) size requirement of a checked bag, which makes the whole offering mute if this size restriction would indeed be applied.
My gut-feeling (but I'm normally traveling with a bicycle, not a golf bag) is that the Swiss FAQ website has an error / typo.

(1) Special baggage regulations are applicable on LX

Without them expressly writing it, I would assume that golf bags are exempt from the 158cm rule (similar to bicycles).

What I tent to do during check-in: try to avoid the word "baggage" / "luggage" when indicating that you'd like to check-in something to the agent. "Hi, I'd like to checking my golf equipment / golf bag / ...".
By my gut-feeling this already steers most check-in agents (especially the inexperienced) into the right direction and to not even look at the standard baggage rules but navigating to sports equipment right away.
