IIRC we used the CIA fact-book, which listed every country, territory and dependency in the world. But there may be a more accurate source nowadays. Maybe the UN publishes one?
How people count countries varies a lot.
I have 194 on my list, including 3 that are not members of the UN and several whose status as a country is not unanimously agreed.

I beleive there are several people on FT who have visited 150+ and many in the 100+ visited as we found out when FT created the badges and people complained the top one wasn't big enough.
