Some upgrades are also taking place in the USA too believe it or not! You may think that they send the engines to wherever is nearest however it seems the engines have just been going to whichever engine shop has capacity. I've seen BA engines in Singapore, El Al engines in the US - It's a huge operation right now.
Spare engines have always been positioned around the world. Cathay Pacific used to have spare RB 211's or Trents in LHR/FRA/SYD/YVR and of course Haeco in HKG so that if an engine required replacing there was one nearby ish.
