A good system shouldn't have a way to be implemented "improperly". Remember iPads and iPhones when those first came out? Three year olds could pick them up and figure out how to use them.
Well, yeah, for the basic functionality. But when you start drilling into the various settings of modern iOS/iPadOS devices, it quickly starts being less and less “3 yo territory”.
I take it you don’t have experience designing/building//implementing packaged enterprise apps.

If you are referring to a recent engagement of Accenture due to the melt down, without having any inside knowledge I would guess there is about a 0% chance that the implementation of Salesforce was triggered by that.
