I have never heard of filing an FAA report for using a seat belt extender.
I always ask for a seatbelt extender when I get the poor man’s first class. So I can make sure I will be safe when I am sleeping.

The odd thing is that the purser wanted to “report” me to FAA for violating safety regulations about how I used the seatbelt extender.
Thats why I am very curious about the FAA regulations regarding seatbelt extender.
