Fare difference could be more or less than the segment upgrade price. No, you cannot swap seats mid-flight, that would be a loophole to drinking too much of The Mascot, for one.

Not true at all. International upgrades clear in advance all the time, and often all but a couple of seats will be cleared around 1-2 days before departure. It entirely depends on how many of the seats are being purchased for cash.
