I ended up calling United this morning. 35 minutes on hold and 3 different reps later, I was offered 10,000 miles as a goodwill gesture, which I accepted. They agreed that the one-time pass from Chase should have been accepted without any question. Pretty sure they figured it was faster and easier to simply offer me 10,000 miles... instead of actually solving the issue of employees not following UA's own policy.
