Not sure what JAL's issue is but they need to improve their IT. ANA isn't much better but I've had fewer issues with them.
The JAL Card issuers have a tendency to mess up things when they think that the information submitted does not match what is on your existing JMB account. MUFG tends to just create a new JMB account for you and associate that with the card. Without even letting you know. The solution to that is to cancel the card and apply again.

Other issuers have other stunts, but managing to mess with the name in the original JMB account is not one I have heard before.
