Just had another look at my boarding pass. Maybe the system rejects my documents because my first and middle name are Thomas XXXXY, but the boarding pass itself cuts the name off to Thomas XXXX (no Y)?
My boarding passes generally always truncate my full name, but I've never had this be a problem for claiming missing mileage.
