The other big point lots of us noticed from the outset and which several people above mention too is the lack of meaningful integration with partners - there's been no real effort to create a 'world of BA' style infrastructure around BAC. Regardless of our likes and dislikes around the programme the real surprise to me has been how bad the commercial implementation has been.
This plus the complete failure to do anything interesting with the milestone bonuses. Why aren't they offering a free CCR visit, a free ET>CE upgrade, £20 to spend at the High Life Café or just something a bit more interesting than a fairly miserly amount of Avios? The point of milestone bonuses done well is to give me the 'this is what you could have won' from the next status up and encourage me to put more spend your way, £30 worth of Avios, an amount I could secure through a few months shopping at Sainsbury's, or a Wine Flyer order, really doesn't do that.
