Unused upgrade vouchers were returned to my account, so this part looks like it actually is automated. It is the ticket issuing one must be like a hawk and make sure they do as soon as status page indicates an upgrade is "confirmed". Any text or green marker indicating it is OK should be ignored until there is a new e-ticket in the mail.
New E-ticket is a must and if it is not received, a call to call center needs to be made.
As an example, from Paris, taxes are different for Economy and Business. Sometimes, the system will be waiting for a payment to be made for the e-ticket to be reissued even if the upgrade is accepted.
