As a doctor, I can't find in the article what "protocol " AA deviated from that would have made a difference in this gentleman's life.

The biggest difference I see is if the couple would NOT have boarded the flight from Miami to Madrid so that the man would have had access to health care in Miami. Looks to me that this was the major error and they made it, not AA.
