My old boss was telling me how he had a job that took him to Brazil a lot. The guy at the company sorting the tickets was known as Roger the Dodger. My boss told me that frequently their tickets would have something like LHR-MAD on the end and he was told just to ignore it. Fast forward to a trip with irrops and the ticketing agent would only put him on the direct flight to MAD, and which point Rog the Dodge quickly had to source him a ticket home.
That an individual traveller arranges an itinerary like this to save (often a lot of) money is fine. That a travel agent books this for a customer is not. The lesson learned would be to make sure LHR is a ticketed stopover (>24 hours) rather than the final destination.
