The AVnerd/geek approach outlined by traumamed, three posts above, does have some added complications. Increasingly, en route aircraft are using digital/virtual communications ("CPDLC") in order to get the next audio frequency for the next ATC sector. So those would not be picked up merely by listening to the correct ATC stream on LiveATC. Even if the next audio frequency were known, it might not be supported by LiveATC. Also, ATC facilities often "combine" sectors during periods of low(er) traffic; particularly in the later evening/early morning hours. So that could further complicate getting the correct audio channel.
Even if a handoff is sent via CPDLC, it is still pretty easy to find the frequency your plane is most likely on via referencing the IFR-high charts as I discussed above. Whether LiveATC is going to have an active stream for your frequency is an issue no matter how you obtain the frequency, whether via good old "United 1234, contact Minneapolis Center on 121.05" or by finding the likely frequency on a chart.
