I book these pretty much every year - going back to the old US Airways days. I as the credit card holder have always gotten the full mileage and full LPs based on the fare that I paid as the #1 passenger. Passengers #2 and #3 got LPs based on the $99 fare.
Ahh that is what is was - this year was my first as the "companion."
