It doesn't matter whether Iberia cares what the U.S. thinks; but they will definitely care that they can be fined by the DOT for failing to make a timely refund when required by Iberia's own terms and conditions.
I filed a DOT complaint against Iberia several years ago, when Iberia was improperly collecting U.S. PFCs on an Iberia Plus award ticket for domestic travel on AA. I was pleased with the resolution:

If you do opt to pursue a DOT complaint, be sure to upload not only your Iberia ticket receipt, but also Iberia's refund policies for your particular ticket type. And make sure that your complaint specifies the type of award ticket which was issued, so that both the DOT and Iberia can see that you were charged an excessive refund fee.
