On Lifemiles, I clicked on "Help Center" at the top of the page which took me to Avianca where I clicked on "Information and Help". Then I scrolled down to "Create a Request" which took me to a page with the heading "What can we do for you?". I scrolled down to "Create a Certificate"and chose "Miles Purchase". Thanks! I opened up a ticket using that method. I also dropped them a Facebook message and they replied within around an hour. Not sure how much help it will be in fixing it but good response time for an airline!
