Same terminal if flying from the US to Japan, and there are no extra formalities other than possibly a document check by JAL at the gate. The official minimum connection time is 35 minutes. It's doable but you will be screwed if there is a delay.
In the other direction, there is customs and immigration and a terminal change from T5 to T3, so it takes longer. 95 minutes is the official minimum; it can be done faster though.
