It's been a while, but unless something has changed, the procedure for E+ seats for refund when you cancel is the refund will automatically go back to the credit card when the scheduled flight leaves without you in the seats. If you want it back sooner, you might be able to do it by applying for a refund online - some have reported success this why. It's also possible an agent, if you are canceling/rebooking on the phone, might be able to take the balance and use it to cover the E+ fee for any new flights, but this is not SOP. Also, with E+ being variable cost, it is definitely possible the price of the seats, even on the same flight, might go up, and technically, you'd be required to pay the new price, but who knows, an agent might work magic and just 0 that out. I wouldn't expect that behavior, but it is possible they could do it.
