There's no such thing as a Chinese middle name, is there? For someone with a Chinese name, s/he obviously can't leave out part (i.e. a Chinese character or its Romanized version) of her/his name. For those with non-Chinese names, or ethnic Chinese who have made some changes to their names in their adopted countries, leaving them out completely is often the better approach because how middle names are presented or abbreviated in various systems.
