I would assume that, like most of BA's IT, the issue is that it's been outsourced and insourced repeatedly and there are so many quick fixes on top of patches on top of undocumented workarounds that there is nobody there with a clear sense of how it works and how tools interact with each other and therefore a lot of 'just do it and see what happens' when changes are rolled out. Ironically, the increase in outages is probably down to them finally trying to get on top of things.
