Both entries are for my Summit Visa and show the same (blanked out) last four digits. I did receive the card on 9/9 and am using it. I did not request a replacement. Aug 29 was the original approval date.
I had something similar in my app, I called a few days after approval asking for expedited shipping. I ended up getting two cards, both with the same card number and CVV, one via UPS and one via USPS.
