SQ do have a process to submit medical things. I just wondered if that was worth it, or whether it would open a can of worms.

I wouldn’t use the process for submitting medical issues to SQ. Just call SQ (perhaps the local office) to put a note in your booking if you want (ie low maintenance option).
When I said “it is not a done thing on SQ to downgrade pax”, it can happen on occasion.
Last month I was flying out of Europe in Suites (revenue) and they discovered after the aircraft landed from SIN that the seat belt on my seat was not working. Plane was fully booked for the trek back to SIN.
I got a call from the local PPS office 90 mins before departure advising me about this and was offered a downgrade to J (where they had some no shows) but also said they would downgrade another Suites pax for me, should I wish to fly Suites (this made me feel uneasy as I didn’t want to inconvenience another pax on my behalf as much as I wanted to fly Suites). They told me to think it over but called me 20mins later letting me know they were luckily able to fix the seat in the meantime.
Long story, but I think you will be fine but feel free to consider my advice from above.
