Incidentally the spacing doesn't matter - Chinese tickets will often show your name as SMITH/JOHNKEVIN
Was your issue that you were simply missing the middle name on your ticket or when you had problems it was because some part of the name was mispelled? I'm trying to book an award with ANA and my ANA account does not have my middle name on it and it doesn't let me add it when redeeming the ticket. Trying to call them now but I'm not optimistic they will easily fix this. My experience with Japan is if you try to do something outside the norm they have difficulty keeping it simple and start making people jump through hoops.
