bj2757, you and I are indeed pretty much in the same boat. As I posted after the program change, I used to be able to make monthly 2,000 United miles redemptions, as well as two Apple and two Best Buy rewards every three months (2 different denominations). I was also able to regularly redeem for 7K Hilton points every two months or so, and an occasional Home Depot, Macys or Target GC was in the mix as well. (Like you, I only received a 1099 once, in a year when I had a nice number of high-return surveys.) All of that is pretty much gone now, and I just plug away at United miles. Every once in a while I'll have a great month or two and be able to grab an Apple GC, but very infrequently. I feel like 24K free UA miles per year is worth a little effort every couple of days, but that's about the extent of my commitment to the program any more.
