Yes, I have seen this before and most likely 3x FlexiRefund in T were cancelled.
My cancelled flight might have initially been returned to P class before then being snaffled by the revenue management system to Y. There was a couple of minutes between cancellation and me checking ExpertFlyer.
Or maybe given that flight was fully sold (ie Y0B0M0…) the system logic is to immediately returns the seat into Y inventory.
