MAD-TFS I'm group 0, the others no group. One of the OWS has fast track, the other doesn't, although we don't need it as we'll be airside already.

TFS-MAD All group 3. One OWS is missing fast track.

Forgive me my ignorance, but doesn't group 0 and fast track extend to everyone on the same PNR?
Group boarding number should be extended to everyone in the booking, however maybe the IT implementation of G0eft this part out (hence others don't have a group number)? G3 for inbound, even for you, could be an IB express not picking up foreign FFP status to correctly issue group number (on going since forever), or didn't implement G0 properly.
Fast track is not extended to anyone automatically.
In theory, for IB group marketed and operated flights
OWE any ffp any cabin any routes will get FT
IB OWS will get FT (same any cabin any routes)
Non-IB OWS will only get FT ex MAD
IB OWS/OWE on an IB marketed flight can guest another pax to MAD FT if guest is on any IB marketed flight. This is not implemented via IT.
