I think unless someone reports that the USB-A ports are broken to the ISM or crew member, no one actually knows they are not functioning and gets it repaired.. I doubt they have anyone regularly going to check every single USB port on the plane to see if they are functioning or not... I know personally I've had power sockets not function and because its a short flight, I just shrug it off and not say anything...
