Meanwhile, I'm getting 184 GBP on Costco Travel with manual transmission vs 274 GBP on Autoslash with automatic transmission. I'm looking at LHR with an intermediate/midsize SUV both through Enterprise and exact same dates and vehicle type. Is this just the cost to guarantee an automatic transmission? If I call Costco Travel, will they be able to "guarantee" or make a note at least for a vehicle with automatic transmission? If I just show up and ask, should I just expect to pay the 90 GBP difference on the spot or possibly even more?
I am not sure that I understand your question.
When I request quotes from AutoSlash, I specify "Automatic Transmission Only," as my travel companion doesn't know how to drive a manual shift car and I am not about to teach her. I am then asked what credit cards I have, affiliations & memberships, such as AAA, AARP, frequent flyer programs and warehouse clubs. AutoSlash then shops for the best available discounts and provides me with quotes, from which I select and book.
I no longer bother to book through the Costco Travel website or check the price of a manual transmission car.
The email confirmation guarantees me an automatic transmission mid-sized car, a mid-sized SUV in this case.

When checking the Budget confirmation number on the Budget website, it showed that the Costco discount had been applied and it reminded me to bring a Costco membership card when picking up the car (I have never been asked to show it).
For what it is worth, for this rental in Italy, we ended up with a new, 2,400 Km on the odometer, fully equipped - GPS, parking sensors, lane keeping and following assist, adaptive cruise control, etc. - Hyundai Kona.
I have never experienced an issue having one available.
I then make a tracking request through AutoSlash and they inform me if and when there are better deals.
