How about using a prepaid US SIM such as T-Mobile EasySpeak along with Gorilla Mobile? With Gorilla Mobile calls to Canada are 5 cents/minute, so your total per-minute cost would be that plus whatever your prepaid SIM airtime is.

I've been using Gorilla Mobile for the past year to call Europe on my cell phone and have been very pleased with their service and rates.
