AC has also launched a new route to NAP from YUL and you may have more space on that. It's fairly easy to fly within EU or train.
I did book a return from YYZ to Italy and I got it for less than $3K, outbound to NAP in LH on the 747 top deck and return in PE which I will hopefully get upgraded on.
There were the odd flight from YYZ-NAP for $1600 on LX on the outbound.
In terms of booking your sister, maybe add her to your profile. My entire family is on my booking profile as is my sister in law and I have never had a problem booking anyone from my profile.
You can also do YYC-VIE round trio in J on those dates for 245K points using PR and take a connecting flight to Italy which should be around $100 o/w
Also YWG-NAP round trip in J on these date for 270K using PR
