IME, this has always been the case when pre-ordering the Japanese meal with United (and legacy United). I fly the Japan routes regularly; occasionally you'll be offered a choice of bread, but never a separate appetizer course off the western menu. Was the same pre-COVID before pre-orders were normal but you could still call in to order the Japanese meal.
