I believe on PMUA back in the day pre-merger, the double ding was 18,000 feet. I don't recall if it changed to 10,000 specifically as part of the UA/CO merger alignment, but it was around that time. I don't recall the PMDL or PMNW policy. Of course, in those days, ORD-PIT was one hour in the air on a 752 and had a full beverage service in all cabins.
I remember it because back in the days of Channel 9 on UA, while en route to ORD, it was around the time we'd get handed off to various sectors of Chicago TRACON.
