With that said, you may run afoul of other issues with a minor traveling internationally by themselves. There are additional links with forms on the UA page. You may need to sign a letter of consent (both parents) for the child to pass through immigration in/out of the US (which they would do at ORD in your example). Our son is going on a group trip to Belize and we were required to sign forms (notarized) that allowed him to go with the group. In that case, we were designating people who he was traveling with.
