I wonder if this is a result of jamming together an outbound booked at the point of release and return flight, again booked 15 days later the night they came out, into the same PNR using a companion voucher. It probably feels to be enough of an edge case for BA that actually putting effort into fixing this is not really worth the bother.
They are actually 2 separate tickets I think. Sometimes they have been under the same pnr, sometimes different.
