Yes, the agent always goes and checks with the Tarriffs Desk and then comes back with some type of outrageous quote.
Maybe all I needed to do was post on Flyertalk because this morning, I called once more and found an agent who told me it would be an even exchange. After around an hour's work, she was able to change it although she did charge me a change fee of 200 EUR per pax.. which I don't think I should have had to pay (I thought the Italy-origin tickets did not have a change fee, and I certainly don't remember a change fee in the fare rules when I bought this ticket) but that was the most reasonable cost I'd had so far so I just went with it.
