As previously mentioned the official policy may be different, but in my experience, young kids are never counted as guests in Doha. As a Gold (Sapphire) member, I see this consistently with CASAFlyer Jr and Ms CASAFlyer. And when I guested in a full family of four, in addition to my own, passes were only deducted for the adults in the large party (one pass for three adult guests and three children, I was Platinum/Emerald at the time, so I had two guests included).
