Does the 200 HKD fee cover your entire stay, or a per day access fee?

As far as I know, expectations around facilities are not hard and fast because things can be out of service and hotels operate independently. I've stayed at a few Marriotts that do not have their own gym facilities and instead offer them in partnership with a separate hotel (e.g: Le Méridien in Kuala Lumpur does not have a gym, instead it provides guests access to the gym at the Hilton next door).

Four Points specifically is a "Select" brand which is the no-bells-no-whistles category. Looking at the Four Points website, there doesn't appear to be any mention of facilities which suggests they're not a part of the brand standard. A good place to understand how brands are managed is in the Franchise documents that Marriott publish. The Four Points document includes the following:

Four Points by Sheraton hotels (“Four Points hotels”) offer high quality accommodations and related services to the traveling public. Four Points hotels offer timeless design, accessible technology, comfort, and convenience and target family, leisure, and business travelers. Hotels typically range in size from approximately 90 to 250 guestrooms, and are generally located in urban or suburban markets. Most Four Points hotels feature warm and inviting public spaces, including a Checkpoint (front desk), pantry, meeting space, and upgraded exercise facilities. Four Points hotels generally feature a restaurant and bar, although food and beverage options may vary by location. All franchised Four Points hotels in the United States and Canada are part of a single system
So it sounds like, while exercise facilities are typical, they're not required. I think that means offering paid exercise facilities in partnership with a nearby hotel is permitted. The document also talks about how hotel facility plans must be submitted and approved by Marriott so I think Marriott must be aware.
